It was the 2013 GF. Lake got 2 weeks, increased to four because it was the GF, down to three with a guilty plea. It's at 0.47 in the video.

It happened three times that season (not all elbows).
In round 3, Ryder got three weeks for concussing Elvis and in round 16, Embers copped three weeks for cleaning up Clancee Pearce.

I remember one game against WB at Etihad where Pav was intentionally hit high after completing a mark and taking 3 long strides. He was forced out with concussion.
Shortly thereafter, Walters was taken in the head while having his head over the ball that was running along the ground. He was taken out of the game with concussion.
It was round 5, 2015. The opposition was actually Melbourne @ the G.
